Obituary for Jeffery L Frazier
FRAZIER, JEFFERY L., 57, of Louisville, died Tuesday June 3, 2014 at Norton Hospital.
He was a Catholic by faith.
Survivors Include his mother Judith Frazier three brothers John (Glenda) and Phillip LaMasters (Yvette) and Bryant K. Frazier (Shirley); two sisters Pamela Simms (Vincent) and Alelia Harish.
His funeral service will be held 11 a.m. Saturday June 7, 2014 at Hazelwood Center Chapel 1800 Bluegrass Ave., with burial in Louisville Memorial Gardens East..
In lieu of flowers the family request that you to bring children's books for Hazelwood Center.
The family acknowledges and thanks the Hazelwood Center and staff for the wonderful care they provided to Jeffery.
